Home-Based Primary Care (HBPC) is a home care program that provides comprehensive, interdisciplinary, primary care in the homes of veterans with complex medical, social and behavioral conditions for whom routine clinic-based care is not effective. HBPC targets patients with complex, chronic, progressive disabling disease and provides comprehensive longitudinal home care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
